Get Doctor’s Prescription
Get Evaluation from an SLP
Give info to a Medicare Participating Provider, they will file the claim. If there is not a provider located in NC, contact the manufacturer for a list of distributors out of state that handle claims for NC.
If there is not a Medicare Participating Provider, you will have to purchase the device and file the claim yourself to be reimbursed at the Medicare approved rate with all deductibles and co-pays applied.
Make sure you get a written receipt that clearly states what was purchased and the cost.
Get a Medicare claim form #1490 by calling 1-800-583-223
Fill out the claim form including the diagnosis in item #4.
Attach the doctor’s prescription, evaluation, and receipt to the completed claim form.
Mail the claim form with the attached information to the address on the form.
